First! Download the SVN-TRUNK version of the code here (http://www.asterisk.org/developers/get-source) and use that as your base. There are several versions of Asterisk (http://www.asterisk.org/asterisk-versions) that are actively maintained, but SVN-TRUNK is the best place to start if you're looking to help.
Then, write your bugfix patch, documentation, or feature. Next, sign up on the bugtracker (http://issues.asterisk.org) - this is required! You'll need to electronically sign/agree to the code submission agreement before your patch will be included.
Open a new issue on your patch, including the code in "diff -u" format.
So if it's a radically new feature that touches a lot of different things in other files, you'll probably want to discuss it on "asterisk-dev" which is the mailing list found here: http://lists.digium.com/ but if it's fairly stand-alone, it may just be accepted as-is.
Some new features are often not included in the main distribution, but are in the add-ons category - there are so many niche features that often the risk of including subtle bugs does not compensate for the additional usefulness, though we still want to include the code.
Documentation is really desired! If you want to write docs, or improve the clarity of the existing docs, this would be great.

0Testing!
We have a big need for people to test patches or new features. This can be trivial or (more likely) is more complex, requiring some understanding of how call flows work, and often how SIP works. Many of the new features in Mantis languish because nobody other than the original developer tests them, which means they're often stuck in limbo forever, which is too bad. Our "long tail" suffers because of this lack of testers - give us a hand, and learn something while you're at it!
